I've solved this and now have a working mythtv setup, the overall dist upgrade worked fine, the OS etc upgraded fine.
The mythtv issue was deeper than just the config.xml. During the schema upgrades there were duplicate records in the tables that the schema upgrade faulted on. It took a long time to figure out the right location for the mythtv- setup.log, once I found that I could see the failure point and take steps to correct it.
