Upon further testing it appears to be a matter of playback and not transcoding. I rebuilt mplayer on my notebook with the mythtv patch and it plays the affected files just fine. I had suspected this since I still get the occasional stutter even on live or recorded MPEG2 streams from my PVR350. Unfortunately the stutters are rare and transient for MPEG2 so I can't give you an easy way to reproduce them other than using the MPEG4 sample I posted.
