Modified Files:

    avformatdecoder.cpp videoout_xv.cpp videoout_xvmc.cpp
Log Message:
Disable the stream_remove part of Mark Anderson's patch. Breaks normal XvMC
on channel change for some reason.
Tweak other stuff."

Just to confirm, that commit fixes the problem.

There is still the occasional: "XvMCPutSlice: This context does not own
decoder!" error, if the section marked "Doesn't seem necessary" is left
commented out. Although it doesn't seem to be fatal and the picture recovers.
