Ok, That's what I thought at first, and I upgraded from MySQL 4.0.15
(which I used for a year happily) to 4.0.23 just in case that would fix it.
Obviously since it happened again I wasn't that lucky :)
Did you upgrade QT after that?
That seemed to fix a similar issue for me.
