Your timeout values may indeed be too short. At one time, it defaulted to ~1.5
seconds, if I remember correctly, which is just too short for a diseqc switch
  and CAM. This default is currently 60 seconds - But is only updated when
recreating the card from scratch.

The configuration setting is available in mythtv-setup, under capture cards /
Card / (signal + channel timeout).

I don't think that's the issue here. I've set the timeout to 8 seconds. Everything tunes, the picture is shown and then after eight seconds has expired I get the dialog appearing. (Apart from which it used to work fine :) )
