Can MythTV output to the PVR-350's decoder output channel?
Yes.
Can it overlay text and images over a playing DVD or live TV?
Yes.
I'm wondering if it's worth getting the 350 rather than the 250 or 500.
Unless you've got a video card with TV-out that you know for sure will work, I'd say the 350 is the way to go.
What about driver support? Anyone have any experience with this device?
I, and many other people, use it regularly. You end up using the ivtv framebuffer device driver for X. It's too slow for games, but works very well for TV. I highly recommend you use S-Video over composite output, though.
