It seems to depend on the particular XBox that you have really, or more accurately, the hardware version.
The version 1.0 XBox came with a heatsink on the GPU (the nVidia graphics processor) that had a small 40mm fan on it. I found this to be very noisy. Version 1.1 (and perhaps later?) units don't have the fan, they have a larger heatsink, thus leaving only the 60mm fan on the back (which is also a bit noisy). I've cracked open my XBox a few times and I've removed the GPU heatsink/fan, replacing them with a Zalman northbridge heatsink. It's definitely an improvement.
