What are you using as the line out. I am using the phone sockets from he svhs/composite cable which just works.
What are you playing? Is it live tv or a captured mpeg? Things to try are Capturing video/audio from /dev/video0 and playing it back with mplayer to see if there is audio in the stream. Playing a captured mpeg that you know has audio at /dev/video16 and seeing if you can here the audio. Assuming the problem is with the capture make sure you are using ivtv's version of msp3400 and not the one that came with your kernel.
