It is not there when viewing the Myth menus.

I get a similar effect at the top of my picture. I assume this is when
watching Live TV (or a recording)? I believe you are seeing the VBI
data.
Did you recently change the versions of the IVTV driver? I didn't see
this
from version 0.1.9 but when I went to 0.2.0 it appears to be in every
subsequent version. You could try to get rid of it using the OverScan
setting of the nVidia driver. HTH!
