Your tuner type is probably incorrect.

There was a bug with the Video4Linux code (I don't
know if it's still present, but it was there quite
some time ago, I know that), where an NTSC card wasn't
properly autodetected, and would be configured for a
PAL tuner.  The way around this was to pass in a
parameter when loading the driver.
