There were two questions to my original post that were unanswered:

1) Is my pvr-500 working?

Yes and No. 
        Yes, it is working in because I can record and watch video and audio
coming in through the composite and audio in connectors. I use the tuner on
a VCR and output the audio and video to the pvr-500. I know this defeats the
purpose of having a dual tuner cards but I wanted to make sure the ivtv
driver worked correctly.
        No, it is not working because the tuner on the card is not supported
by the tuner.ko module and there is no way to change the channel on myth. I
suspect it will not be long until this tuner is added to the ivtv tuner
module. If it helps, it appears to be a Philips FQ3236

2) How did I use the dvb-apps for testing?

Notice that it has the FE_HAS_LOCK. I have an outdoor UHF antenna mounted
inside my attic and pointed towards the city.

Then I started mythtvsetup and noticed that it would not detect the card. It
should have shown up under the dvb 0. It turns out that even though I had
created the devices /dev/dvb/adapter0/frontend0 ... they disappear after a
reboot. So I edited my rc.sysinit and added a line that executed 

/usr/src/dvb-kernel/MAKEDEV-DVB.sh

Now mythtvsetup would see the card but it would not recognize it by name. It
turns out that mythtv 0.17 has a bug and even though on the website it says
that it supports the air2pci, it really does not. You have to go and compile
the latest CVS version. Is there any apt-get of mythtv-suite created against
snapshots of CVS? I compiled mythtv without much difficulty but I really
want all the plugins and it is very time consuming to get all the
dependencies for them.
