Set the IEC958 P volume to 0 in alsamixer.
Make sure you see the red light when you remove to cover on the TOS connector. Might be your receiver if you can see the output.

Yay! Got it working now, the 'Analog to IEC958 Output' should have been off. I did try that one at one point but then testing it with aplay -D spdif file.wav didn't work. As it turn out aplay file.wav (without the -D spdif) worked perfectly.
