Yup, I'm using a PVR-350 for TV-Out.
Now why would they do that? I'm assuming there's a technical reason for it.... ?
How do you like your 5200FX?

Are you using a PVR-350 for TV-Out?
When I was using my PVR-350 for TV-Out (around MythTV 0.16 or so), you could only get the semi-transparent program guide (with no corner window), the regular one didn't work.
I would guess that it's still the same in MythTV 0.18 (but I don't know for sure since I'm using a GeForce 5200FX card for TV-Out).

Hmmm... Doesn't do it on mine. (Knoppmyth V5R15.1). On mine, the program guide simply stays Semi-transparent all the time and has no corner window. I'd much rather have the guide become opaque with the corner window - much easier to read the guide.
Is it just possibly a question of Knoppmyth not using the lastest version of MythTV? (Not sure what version it's currently using...)
