Craig, Is anything loading tveeprom for you? I'm using ivtv-0.3.5p with the PVR-150. I've done much the same as you and placed
/sbin/modprobe tveeprom /sbin/modprobe ivtv in my /etc/conf.d/local.start file. (gentoo) The tuner seems to be working fine.
