I just ran into something like this and here's what ended up working for me.
Run mythtv-setup and look to see if there are channels that say unassigned.
If there are exit mythtv-setup and run it again this time answering yes to
"delete my channel settings". You will also need to setup your video
sources and input connection again. After this, at least in my case,
mythfilldatabase then worked. Also, the 401 error on the first attempt
seems to be normal.

I'm trying to setup Mythtv 0.18 for the first time. I have gone through
all the setup instructions, and I'm at the point where I need to run
mythfilldatabase. The problem is that I continuously get 401 errors.
I searched the lists, but the only solution I've seen is run mythsetup and
to clear the credentials and re-enter them. I did that a number of times
with no success. I verified that the logon data is correct by runing
tv_grab_na_dd and pulling down the xmltv data. I even tried generating a
new logon (hey I'm getting desperate here), but the results are always the
same.
When I run mythfrontend I can see the data direct expiration date. Doesn't
that mean that mythfilldatabase was able to logon?
Does anyone have a clue what the problem might be? If you need more data,
just let me know and I'll provide it.
One more thing I've tried using mythfilldatabase 1 -1 tv_grab.xml to load
the database manually. After reading the data for a few seconds
mythfilldatabase tells me not to use tv_grab_na_dd. Isn't there any way I
can load the channel/program information into mythtv that it will use?
