I was having similar problem and find a hack to generate the .mp2.

I believe avidemux2 either doesn't like the audio encoding from my .nuv (Don't know whether it is because my PVR-150) or avidemux2 has changed some behavior. I need to split the save-raw-audio and save-raw-video in 2 seperate run. e.g. first one only -save-raw-audio and the second one does -save-raw-video
Now I can generate both files.  I haven't yet try the next step yet.
