For anyone following the thread, once "Use video as timebase" was disabled, everything worked perfectly. The trick was to have everything with AC3 passed through by Myth and to have everything else sampled to 48000 Hz for output via PCM. ALSA made this second part possible. Thanks for everyone's help.
