We have been using 2.0b4 for quite some time now. What documentation are you looking at? The docs included with 2.0b4 and the ones here http://nagios.sourceforge.net/docs/2_0/escalations.html have mention of first and last notification. Also, taking out the notification_interval causes Nagios to behave as if the notification_interval was set to 0. Upon escalating it only notifies once.
