Either you did not completely follow the documentation on enabling external command and your web server is not in the nagios group (and restarted), a directory above /nagios/var/rw/nagios.cmd does not have sufficient permissions to allow your web server access to that file or you have SELinux enabled and it is blocking access to that file from your web server. Those are the most likely problems given the information you've provided.
