Always test as the nagios user since that's the user the plugin will be run as and certainly has fewer privileges than root. Also test the plugin exactly as you have it defined. If you do so, you'll see that nagios is trying to run -- /path/to/libexec/check_myslqd.pl -H <somehost> -u -p You don't define $ARG1$ or $ARG2$ as part of the service check_command, so they have null values when the plugin is called. I'll bet it doesn't like that and complains loudly.
