We've got a problem with Nagios 3.0.4 running inside a Xen VM. Nagios version is 3.0.4.
It seems to hit the Max concurrent service checks value of 300 and then just keeps repeating that in the log continuously, but if you look at the nagios processes, there are usually less than 20 including check_* and nagios processes themselves. I know it's an instantaneous value, but I would think that if it's continuously hitting the max, I'd see more than a couple handfuls in the ps output.
