Use NRPE (active) or NSCA (passive) for the check result submission.

Write your test routine, house it on your remote server.

For NRPE, use 'check_nrpe -H <asteriskhost> -c <commandname>'.  Have the NRPE 
daemon listen on your asterisk box with the '<commandname>' defined in the 
'nrpe.cfg' file.

For NSCA, the NSCA daemon listens on your Nagios server, and a cron-job runs 
your check and submits the results.
