Nagios itself doesn't use anything in the database. I would point you to the NagioSQL support page at http://www.nagiosql.org/support.html , but it seems to be down at the moment :-( I doubt NagioSQL would need much in the nagios_externalcommands table so I guess you could trim it quite aggressively. I don't use NagioSQL myself, so can't be 100% sure though.
