Off the top of my head.......

......did you see the "shattering"? Certain problems will present themselves
in certain ways. By that, what I mean is that not curing your whites
properly will cause a service breakdown that looks different than, say,
applying your product too thickly in the sidewalls. Tell us more about this
shattering:)
......was your prep spot on?
......was the dust removed properly when it had to be removed?
......is it really COLD where you are? I lost all my enhancements over the
holidays because it was constantly in the -40s and no amount of cuticle oil
could keep them from drying up like popcorn farts, even in doubled mittens.
......it could be your light unit, even if it was new, maybe the lamp
(bulb) needs to be changed.
......is it the right wattage for the product?
......too long for the client?
......maybe not enough reinforcement in stress area?
......maybe your client got bored and tried having a little finger snack and
found out the hard way that gel doesn't bite well?
......what fingers? Sometimes that is a clue.

There are many, many reasons. Take pictures if you can, so we can see.
Sometimes we can tell right away by looking at the damage.
