Is she a runner?  I have seen this happen to runners, or someone wearing
"hard Gel" on toes and not shortening them. As they grow out the toe nail
doesn't have enough flex, so eventually it will lift and separate from
repeatedly hitting the end of the shoe.  Most times the client doesn't even
know it's happening until it's too late. But this seems to be in extreme
cases.  The two clients I have had this happen to are runners.  Once we have
shortened the toe nail and convinced them to either come in more regular to
have them shortened or to even go so far as to shorten them themselves
between appointments the problems solves itself.
