   The list of Lower Elwha Klallam tribal members waiting for decent
housing is more than 100 families long. Yet 10 brand-new homes sit
locked and empty on the impoverished reservation outside Port
Angeles because of a missing two-page federal form.

   The Department of Housing and Urban Development (HUD) has frozen
funding for the project and demanded repayment of $1 million spent
on it so far, all because the tribe didn't file the appropriate form
before starting construction.

   "This will bankrupt my tribe," said  Lower Elwha Klallam housing
director John Williamson.

   In Idaho, the Coeur d'Alene tribe faces a similar situation, with
HUD poised to seek a refund of more than $3 million for the same
alleged paperwork violation.

   The form in question certifies that a tribe has completed an
environmental review of a federally funded housing project before
starting construction. In both cases, the tribes did the
environmental reviews but neglected to submit the form.

   HUD released the funds anyway and didn't discover the record was
