I'm sure that I'm not perfect, and pretty sure that no one else is either.
So if we each have probability p of making a mistake, and we work together
in some n-fold fashion, we have approximately probability p^n of a mistake
getting through. This number is never zero.
I do think that for any team T, their quality can probably be higher than
it is, and that the most efficient approach to development comes very close
to the zero-defect level.
