GraphAware has created a ChangeFeed extension that exposes change sets via a REST interface: http://graphaware.com/neo4j/2014/08/27/graphaware-neo4j-changefeed.html
An extension could also deliver them in a continuous stream, to allow something like triggers. But it might be even better if the change stream were delivered over Bolt.
