Here's my artwork for the original incarnation of this circuit. You
can omit C5, C6, R7, & R8. They're just an extra bias voltage (+90V),
you probably won't need.

Component side in green. Solder side in red. Where the overlap, it
turns yellow, but remember, that the copper is on their respective
sides.
