I do an rpm -qa and come up with: perl-DBD-MySQL-2.1021-3. See my MySQL is
installed via the tarball but the DBD::mysql is installed via the RPM. I
wonder if there is a library out of place or something.

Have you installed the MySQL client/libraries and the DBD::mysql Perl
module?
