"holes" == open ports?

It's hard to say what the cause might be without further information.
For example, which port scanner(s) are you using in your Nessus scan and
how do you have it/them configured? Have you looked at the network
traffic to see what Nessus is sending and receiving from the targets?
Are you running nmap from the same host as Nessus? ...
