This is only true for conf files which check the m2d files. Currently, that's
only the mib2c.mfd.conf being developed for 5.2. You'll need to use CVS code if
you want to try it before then.

Both. But be careful - re-runing mib2c will overwrite the old code. If you want
to be able to re-generate code without losing your changes, you should use the
new utility mib2c-update (also new for 5.2).
