That is the correct place for the MIB (text) file. The code for the MIB
module (C files) should be placed in the agent/mibgroup directory, and then
configured into the agent. This is covered in the FAQ.

That depends on how you implement the MIB module (C code). It depends on the
mib2c conf file used, and how you modify the skeleton code.
