Cheaper agents don't implement support for all objects in a MIB
because to do otherwise would be more expensive.  There is no
technical reason why createAndWait is bad from a modeling point of
view.  It's only because it's more of a pain to implement.

createAndWait is needed, for example, on tables with a very large
number of objects in the table (another questionable design practice,
to be sure, but hard to avoid in some situations too).
