Based on the cisco logs and the ethereal analysis (which doesn't show source IP addresses), I would bet the TFTP server is unable to serve up the requested file.


To test this I would get SNMP out of the way - just try to TFTP file from from the IOS CLI to a temporary file on the flash. If the copy is successful then I'd suspect the SNMP request. If the copy is unsuccessful then the TFTP server is not up, the path for the file is incorrect, or there is a permissions problem with the file/directory on the TFTP server.
