I finally got around to testing your patch, but it didn't help.
I traced the traffic between the Net-SNMP agent and Squid and what I see
is the agent sending Squid a getnext on my last OID (in my MIB) 6 times,
Squid sends NO SUCH NAME each time.  After 6 times the agent gives
up.  Meanwhile the walk times out.

The patch I got from the Squid guys has me up and running, but I'll be
happy to try out any other patches you send if there's something you'd
like to try.
