What net-snmp version are you using? What's the output of net-snmp-config --snmpd-module-list | tr " " "\n" | grep ip-mib ? Unless the CentOS package is more recent than I think it is, you likely need to drop the CentOS package and build 5.4.1.rc1 yourself from source.
