If you issue the command snmpset ...... .1.3.6.1.4.1.2021.255.8.1.8 i 123 (assuming that this MIB object is defined as an integer) then your script will be called as /home/monitor/tests/extensio.pl -s .1.3.6.1.4.1.2021.255.8.1.8 integer 123 It's then up to your script to check that the OID, type and value are OK, and accept or reject the request accordingly. See the section on the "pass" directive in the snmpd.conf(5) man page for details.
