struct uvideo_stream: broke out isoc fields into separate struct
struct uvideo_stream: added union with isoc or bulk fields
uvideo_start_transfer, uvideo_stop_transfer: use stream-specific functions
uvideo_stream_start_xfer: added function; has room to do bulk transfers
