It describes very similar symptoms and suggests disabling IPv6. This seems to have fixed it! Package installations are fast now.
It looks like with IPv6 enabled (the default), pkg_add/pkgin try several v6 addresses for cdn.netbsd.org, with a long connect timeout each time.
