Check your CODEBASE specification. It should tell where the Applet class
files or jar are kept, RELATIVE to the Web Root.
Also look at the Java Console to see what is happening.
View -> Internet Options -> Advanced -> ..
somewhere deep down you will see 'Java Console Enabled'. Check it on.
And then do View -> Java Console
