If you do a refresh the values you add will definitely disappear.
You could try to add your values last thing after a call to doAutoFill in
the onBeforeDisplayEvent for the field.
Two ideas you have probably already thought of if you can't get a real
solution:
1. Fill the combo completely by hand.
2. Change the SQL to be a union with the values you want to add (I
think this one is cute).

I am having difficulty adding a value to a combo box that is
populated by a DO. I
add the value after the combo box has been populated by the
DO using the addSelectable()
method. However, the value does not display in the list.
If I refresh the combo
box using the method autoFill(), the value I'm trying to
add just gets overwritten
(or I think at least). Any suggestions?
