Don't know if either of these is your problem but:
There's an HTML error on the </HEAD> tag.
You don't need the </FORM> tag. ND will supply that for you.
You probably should turn on verbose logging in the CP service and see what
that tells you. Without seeing the entire HTML template, it's hard to tell
what other problems may be occurring.
