1.2.3 is the one on RH 7.2 right?  Anyway, yeah you can.  I just did it.

You need to grab a current cvs snapshot, and the 2.4.18 kernel.
Run make patch-o-matic in the userspace directory of netfilter (with
your linux source installed in /usr/src/linux)
Say yes to every patch that has submitted/ in it.  I think one will warn
about not patching cleanly, but that's ok, just make sure not to patch
it anyway.  Make sure to apply the 0-newnat13 patch and the h323 patch.
There are others, and you may want to use them.  Be careful not to apply
any patches that warn at the bottom of them that you need to recompile
the userspace tools (since this is exacltly not what you are wanting to
do). Then configure your kernel, I'd compile all the netfilter stuff as
modules.  Compile, Reboot, and load the conntrack_h323 module and the
nat_h323 module and you'll be on your way. Good luck.
