I'm assuming that to set up an MCU I'd need a much more sophisticated
kit than an ILS.  Can anyone steer me toward a review of the relative
merits and problems of each?

NetMeeting audio/video is one to one -- use of an external MCU
 conference server) will allow many to many audio/video ( for small
number of participants). An MCU could be configured to do what you
want - allow one party to send video and audio to all the others
 again for small numbers).
