It can be number of issues, driver, wpa_supplicant , NM, ... Are you able to connect just with wpa_supplicant? To debug further, please grab the following info: 1) nm-tool output 2) NetworkManager logs (/var/log/messages, /var/log/syslog, ...) 3) wpa_supplicant log (/var/log/wpa_supplicant.log, ...)
