Please find patch for Modem Manager plugin version 0.4 to support LTE

List of attachments and explanation:
1) ModemManager.patch : patch file generated using diff.
2) ModemManager.tar: source file which has newly added files for plugin.
Also the patch does not apply without a lot of conflicts to neither
MM_05 or master branch and thus will not be any useful for other than
patching 0.4 versions and not likely to be applied here for future
versions. Please target (rebase to) either MM_05 or master branch.
