What I found out my problem was --- It turned out that there was a "corrupt" 
message in the mail folders. I used Kong to look at the files and noticed 
that there was one more file than Kmail showed as messages. When I opened it 
using Kedit it had a horrendously long string of numbers in the from field. I 
deleted the  file and Kmail got alot better.
Don't know if your problem is similiar, but its something to look at.
