Yes, you're right, permissions problem. I did have this, but it's a long time back, so I'm digging for memory. I think you may actually have to delete the file, as well as setting user permissions, but try just perms first. The other thing, is that efax when used from kword generates a log file, which you can see from the efax screen that asks for fax number. In there you should see the name of a log file, which is likely to start with T then the telephone number. That gives you info about the failure, but in this case it is likely to be similar to your messages above.
