Outlook lets you thread by conversation topic, ie subject, but it 
doesn't see or produce the conversation reference header lines.

The result is a hijacked thread looks fine under outlook, and outlook 
spawns disconnected threads.

(Let me know if I'm wrong, I'd love to be able to do this on my work 
machine.)
